Missed reps
Falling short of a target rep count mid-set doesn't get treated as one verdict. What changes for the rest of your workout depends on how far short you fell. What changes for next time depends on whether this was a one-off or a real pattern — and those are two separate questions with two separate answers.
A small miss (0–1 reps) changes nothing. A moderate miss (2–3 reps) softens your remaining rep targets, but the weight stays the same. A big miss (4+ reps) is read as a real fatigue signal: the weight for your remaining sets drops, and your rep target goes back to the original plan rather than being softened further. None of this touches next session's prescription on its own — that only changes once the same weight has come up short twice in a row.

A missed set can change two different things: what your remaining sets ask of you today, and what next session's starting weight should be. Conflating them is how one rough set turns into an overreaction that outlives the workout it happened in.

OVED compares each completed set's actual reps against its own target the moment you log it, and adjusts the remaining sets of that exercise — not the sets you've already done, and not other exercises.

This applies the same way no matter which set the miss happens on — a bad second set is treated exactly like a bad first set. And it only ever touches sets you haven't logged yet: anything you've already completed and logged stays exactly as recorded.
3. Next time you train this
What happens mid-workout and what happens to the prescription you're given next time are governed by different, deliberately more conservative rules.
There's no prior comparable session to check against yet. This is never treated as evidence the weight is too heavy — it's simply your first data point.
If the last comparable session at this weight was a genuine hit, today's miss is read as a one-off. The prescription holds — it doesn't drop from a single rough session.
Only once the same weight has come up short on two consecutive comparable sessions does a lower weight become a real option for next time. That's the actual evidence bar: a pattern, not a single set.
This is a genuinely separate mechanism from the in-workout response above — it's entirely possible for a session's weight to get cut mid-workout because of a big single miss, while next time's starting prescription still holds, because that one session alone doesn't yet establish the pattern the reduce decision requires. 4. Why the bar is this conservative
Sleep, stress, how the previous session went, and plain day-to-day variation all move performance around without meaning anything has actually changed about what you're capable of.
Reacting to every rough set with an immediate weight cut would mean the programme never actually earns a heavier weight for long — one bad day and it's undone. Requiring a repeated pattern before a reduction is offered keeps the response proportionate to how strong the evidence actually is: a graduated in-workout adjustment when fatigue is visible in real time, and a more conservative bar before that becomes a lasting change to what you're prescribed next.
